After receipt of an questionnaire for accreditation is received, an AHBA representative will contact your office. The complete cost, including the site visit and eligibility of accreditation will be provided. You will be provided a complete list of accreditation requirement plus the survey lists that will prepare you for the site visit by a qualified independent surveyor.
